29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  The employer is failing to protect employees from fall hazards.  This was most recently documented on or about February 24, 2026 when two ironworkers were noted placing hollow core on Digester 4 (Cell #8) of the DVO at height while standing on top of the previously set piece of hollow core.  Employees were exposed to a fall distance of 16 feet to ground without the use of fall protection. Employees had their harnesses on, but were not tied off from the anchor points. An employee fell along with the damage hollow core panel and hit the concrete ground below causing injury to their back and wrist. The employee was hospitalized.
