29 CFR  1904.32(b)(6): A company executive must certify that he or she has examined the OSHA 300 Log and that he or she reasonably believes, based on his or her knowledge of the process by which the information was recorded, that the annual summary is correct and complete. You must post the summary no later than February 1 of the year following the year covered by the records and keep the posting in place until April 30.  On or about February 9, 2026, the employer did not post the OSHA?300A Annual Summary as required.  The 300A summary was not posted in any location accessible to employees, and no posting was observed.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1910.37(b)(4):If the direction of travel to the exit or exit discharge is not immediately apparent, signs must be posted along the exit access indicating the direction of travel to the nearest exit and exit discharge. Additionally, the line-of-sight to an exit sign must clearly be visible at all times.  On or about February 9, 2026, employees working in the operations manager's office, accounting office, storage area, and break room were not provided with a direct and unobstructed path to an exit. To reach the main warehouse exit doors, employees were required to travel through multiple intervening offices, doorways, and hallways, rather than having a clear exit access that led directly to an exit route as required.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1910.38(b): Written and oral emergency action plans. An emergency action plan must be in writing, kept in the workplace, and available to employees for review. However, an employer with 10 or fewer employees may communicate the plan orally to employees.  On or about February 9, 2026, the employer did not maintain a written emergency action plan for employees.  Employees were not trained how to respond to emergencies in the office and there was no written procedure in case of an emergency. 29 CFR  1926.503(b)(1):The employer shall verify compliance with paragraph (a) of this section by preparing a written certification record. The written certification record shall contain the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer. If the employer relies on training conducted by another employer or completed prior to the effective date of this section, the certification record shall indicate the date the employer determined the prior training was adequate rather than the date of actual training.  On or about January 30, 2026, at jobsite 13956 Starved Rock Place, Huntley, Illinois, employees were exposed to fall hazards while performing roofing work and the employer did not certify that employees had been trained to recognize fall hazards, and the procedure to be followed in order to minimize those hazards.  Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
