29 CFR 1910.1200(e)(1):Employers shall develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, safety data sheets, and employee information and training will be met, and which also includes the following:  1. Requirement for labeling and other forms of warning as specified in paragraph 29 CFR 1910.1200 (f) 2. Safety data Sheets as specified in paragraph 29 CFR 1910.1200 (g) 3. Employee information and training as specified in paragraph 29 CFR 1910.1200 (h) 4. a list of hazardous chemicals known to be present in the workplace 5. Methods to inform employees of the hazards on non-routine tasks:  On or about February 6, 2025, employees were exposed to respiratory health hazards associated with Versi Foam System 1, 9, 15, 50 and 1.75 pcf, Component A & B.  The employer did not develop, implement, maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) of this section for labels and other forms of warning, safety data sheets, and employee information and training will be met.

29 CFR  1910.1200(h)(1): The employer did not provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees had not previously been trained about was introduced into their work area:  On or about February 6, 2025, employees were exposed to respiratory health hazards associated with Versi Foam System 1, 9, 15, 50 and 1.75 pcf, Component A & B.  The employer did not provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees had not previously been trained about was introduced into their work area.    Abatement certification is required of this item in accordance with 29 CFR 1910.1903(c).

29 CFR 1910.134(c)(1): In any workplace where respirators are necessary to protect the health of the employee or whenever respirators are required by the employer, the employer shall establish and implement a written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i)-(ix) with worksite-specific procedures:  132 S. Sycamore, Franklin Grove, IL:  On or about February 6, 2025, employees were required to wear 3M Full face respirators for protection against respiratory hazards associated with the Versi Foam System 1, 9, 15, 50 and 1.75 pcf, Component A & B..  The employer did not develop and implement a written respiratory protection program with worksite-specific procedures for respirator use that included all provisions in 29 CFR 1910.134(c)(1(i)-(ix).  This item has been corrected.

Sycamore, Franklin Grove, IL:  On or about February 6, 2025, employees were required to wear 3M Full face respirators for protection against respiratory hazards associated with the Versi Foam System 1, 9, 15, 50 and 1.75 pcf, Component A & B..The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested, or required to use the respirator in the workplace.  This item has been corrected.
