29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a) On February 11, 2026, and at times prior, employees were exposed to caught in hazards from entering the Smart Machine Tool CNC Machine, Model# NL2500SY, Serial# N25YS3221023, to perform cleaning and maintenance, without being protected from accidental start up due to the energy control procedures not clearly and specifically outlining the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy.

29 CFR  1910.147(c)(6)(i):The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  a) On February 11, 2026, and at times prior, employees were exposed to caught in hazards from entering the Smart Machine Tool CNC Machine, Model# NL2500SY, Serial# N25YS3221023, to perform cleaning and maintenance, without being protected from accidental start up due to the energy control procedures not being inspected on an annual basis. The last inspection of the program was conducted on April 22, 2022, and the program was missing energy control procedures that clearly and specifically outlined the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy.

29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) On February 11, 2026, and at times prior, employees were exposed to amputation hazards from operating and feeding the Smart Machine Tool CNC Machine, Model# NL2500SY, Serial# N25YS3221023, that was missing a panel which guarded employees from the moving motor belt (4500 r.p.m.) of the feeder.

29 CFR  1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical.  a) On February 11, 2026, and at times prior, the employer did not ensure that secondary containers such as spray bottles of "tap oil" were labeled in accordance with this standard.  Steelhead Composites, Inc, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.1200(f)(6)(i), which was contained in OSHA inspection number 1515598, citation number 2, item number 1 and was affirmed as a final order on April 1, 2021, with respect to a workplace located at 500 Corporate Circle Ste. O Golden, CO 80401.
