29 CFR  1926.502(d)(16): Personal fall arrest systems, when stopping a fall, shall be rigged such that an employee can neither free fall more than 6 feet (1.8 m), nor contact any lower level:   a- 1 Fairshare Drive, Woolwich Township NJ:   Two workers were not protected from falling when walking and working in and on the top of floor trusses while wearing full body harnesses whose connected lanyards were either wrapped around a connector plate & chord (within a wooden floor truss) and connected back to itself or wrapped around a wooden interior wall frame's top plate and connected back to itself.  As such, these workers were exposed to fall hazards in excess of 10 feet 6 inches to the flooring below. Condition observed on or about 02/11/26.  b - 1 Fairshare Drive, Woolwich Township NJ: Two workers were not protected from free falling when the slack of their lanyards,  attached to their full body harnesses, exceeded the fall hazard distance.  Condition observed on or about 02/11/26.  Mega Contractors, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1926.502(d)(16)(iii)), which was contained in OSHA inspection number 1767354, citation number 1, item number 1 and was affirmed as a final order on September 11, 2024, with respect to a workplace located at 25 East Allendale Road, Saddle River NJ.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
