29 CFR  1910.252(a)(2)(iv): Authorization. Before cutting or welding is permitted, the area shall be inspected by the individual responsible for authorizing cutting and welding operations. He shall designate precautions to be followed in granting authorization to proceed preferably in the form of a written permit.  a. On or about September 2, 2025, for the well site located at grid location 40°19'00.8"N81°12'26.9"W:  the employer did not ensure the working area was inspected and the employer failed to assign an individual to perform inspection, to ensure precautions preventing fire and/or ignition of flammable vapors were implemented prior to employees using a chop saw to cut a metal ladder at an active well site. As a result, employees were exposed to fire and potential explosion   hazards when sparks were generated in the presence of flammable vapors. On September 2, 2025, when an apparent spark from the cutting activity ignited vapors in the work area and an employee suffered serious burn injuries from the resulting fire event.

29 CFR  1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.  a. On or about September 2, 2025, an employee was admitted to the hospital with injuries sustained during a flash fire. The employer did not report the inpatient hospitalization of the employee to OSHA within 24 hours and the inpatient hospitalization was not reported until February 9, 2026.
