29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or around February 11, 2026, in the Aluminum Fabrication Area Of Building 2 - The employer did not ensure that hazardous energy control procedures were developed, documented, and utilized by employees prior to allowing them to perform service and maintenance activities on the PMI upcut/miter saw. Hazardous energy was not isolated prior to employees performing servicing/maintenance work such as replacing saw blades, exposing them to laceration and amputation hazards. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. a) On or about February 11, 2026, the employer did not ensure that employees engaged in maintenance activities such as blade changes on the PMI upcut/miter saw, received training in the recognition of applicable hazardous energy sources and the methods and means necessary for isolation and control. Employees were exposed to laceration and amputation hazards. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about February 11, 2026, in the Aluminum Fabrication Area of Building 2 - Employees were exposed to amputation and laceration hazards while operating a Delta 10-inch table saw (MN: 36-679, SN: 010747). The table saw did not have effective guarding, leaving the rotating blade fully exposed during cutting operations. b) On or about February 11, 2026, in the Aluminum Fabrication Area of Building 2 - Employees were exposed to caught-in and entanglement hazards while operating the Bridgeport Drill Press (SN: J17736) and the Bridgeport Milling Machine (SN: J34454). The rotating drill chucks were not adequately guarded. The rotating spindle, chuck and drill bit were fully exposed during operation. c) On or about February 11, 2026, in the Aluminum Fabrication Area of Building 2 - Employees were exposed to caught-in and entanglement hazards while operating the South Bend 10" Quick Change Lathe. The rotating jaws and spindle were not guarded when engaged in machining operations. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

Pulleys, any parts of which are seven (7) feet or less from the floor or working platform, shall be guarded in accordance with the standards specified in paragraphs (m) and (o) of this section. Pulleys serving as balance wheels (e.g., punch presses) on which the point of contact between belt and pulley is more than six feet six inches (6 ft. 6 in.) from the floor or platform may be guarded with a disk covering the spokes. a) On or about February 11, 2026, in the Aluminum Fabrication Area of Building 2 - Employees were exposed to caught-in and entanglement hazards while operating the Bridgeport Drill Press (SN: J17736) and the Bridgeport Milling Machine (SN: J34454). The belt and and pulleys were fully exposed during operation, allowing employees to reach into or inadvertently contacting rotating components when operating machines or making adjustments.
