29 CFR 1910.132(d)(2): The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment:   (a) On or February 11, 2026,  in the Aluminum Fabrication Area Of Building 2 - The employer had not performed formal workplace personal protective equipment (PPE) assessments with certification, within the facility, to include the Aluminum Fabricator working on the PMI upcut/miter saw.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.132(f)(1): The employer shall provide training to each employee who is required by 29 CFR 1910.132 to use PPE. Each such employee shall be trained to know at least the following information contained in 1910.132(f)(1)(i)-(v):   (a) On or February 11, 2026,  in the Aluminum Fabrication Area Of Building 2 - The employer had not provided personal protective equipment (PPE) training to employees required to wear PPE in the facility, to include the Aluminum Fabricator working on the PMI upcut/miter saw.  The employer was not wearing eye protection required by the employer (safety glasses) and had not been provided on training to include when the PPE was necessary, what PPE was necessary, how to put on/remove/adjust/wear the PPE, the limitations of the PPE, and the proper care/maintenance/useful life span of the PPE.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1910.133(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  (a) On or about February 10 and 11, 2026,  in the Aluminum Fabrication Area Of Building 2 - The Aluminum Fabricator working on aluminum fabrication equipment including a PMI upcut/miter saw and a Bridgeport mill was wearing prescription glasses without prescription safety glasses having side shields or over-the-prescription glasses safety glasses, exposing the employee to aluminum chip eye impact hazards.  (b) On or about February 10 and 11, 2026,  in the Aluminum Fabrication Area Of Building 2 - The Aluminum Fabricator working on the PMI upcut/miter saw was wearing prescription glasses and was not wearing goggles to protect the employee from aluminum fines (air suspended dust) eye irritation hazards.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  (a) On or February 11, 2026,  in the Aluminum Fabrication Area Of Building 2 - The employer had not provided hazard communications training to employees exposed to hazardous chemicals within the facility, to include the Aluminum Fabricator working on the PMI upcut/miter saw.  Employees in this area are or may be exposed to chemicals including, but not limited to: aluminum particulate from Building 2 (aluminum upcut/miter saw), powder coating paint fugitive dusts from Building 1, lubrication oils from Building 2 aluminum fabrication equipment, and migrating welding fumes from Building 1.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

At the end of each calendar year, you must complete the requirements of 1904.31(a)(1) - (4): Review the OSHA 300 Log to verify that the entries are complete and accurate, and correct any deficiencies identified; Create an annual summary of injuries and illnesses recorded on the OSAH 300 Log; Certify the Summary; and Post the annual summary:  (a) On or about February, 11, 2026, with regard to the establishment's OSHA Form 300 for 2025, the employer had not completed the following tasks:  (i) Reviewed the OSHA Form 300 Log for accuracy and completeness, including correcting deficiencies; (ii) Created an annual summary (OSHA Form 300A); (iii) Certified the annual summary (OSHA Form 300A); and (iv) Posted the annual summary (OSHA Form 300A)
