29 CFR 1910.28(b)(1)(i):  Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  a. On or about February 18, 2026, -Cut To Length-LL1 Line 60" Uncoiler Pinch Rolls Area employees working at the leading edge of a pit at the height of approximately sixteen (16) feet were not provided the appropriate protection to prevent a fall.

29 CFR  1910.147(c)(7)(i)(A): Each authorized employee did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control:   a. On or about February 11, 2026, Cut To Length-LL1 Line, an employee sustained a index finger degloving and ring finger fracture while performing the service and maintenance task of wiping down rollers with a rag on the 60" Uncoiler Pinch Rolls Area.  The employer failed to provide training to each authorized employee performing service and/or maintenance to machinery or equipment to ensure the employee acquired the knowledge and skills required for the safe application, usage and removal of energy control devices.

29 CFR 1910.147(d):  The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    a.  On or about February 11, 2026, - CTL-LL1 Line, an employee was exposed to machine hazards associated with moving parts and pinch-points during cleaning of the 60" Rollers at the Uncoiler Pinch Rolls area Machine, which resulted in the degloving of the index finger and fracture of the ring finger of the right hand.  The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the servicing work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)((i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.

29 CFR 1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a. On or about February, 18, 2026, - Cut To Lengh-LL1, the machine guarding (swinging gates)  in the Uncoiler Pinch Rolls Area did not prevent employees from entering hazardous areas, thereby exposing employees to rotating parts and lacerations.
