29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   a) Lots 18-22, Roof: On or about February 17, 2026, and at times prior, the employer exposed employees to fall hazards of approximately 19 feet to the lower ground level, in that a fall protection system was not provided to employees engaged in framing activities.

29 CFR 1926.502(d)(6)(i): Snap hooks were connected  directly to webbing, rope or wire rope:   a) Lots 18-22, Roof: On or about February 17, 2026, and at times prior, the employer failed to ensure proper anchorage of personal fall arrest systems. Employees were exposed to fall hazards to the lower level, in that the snap hook was connected to a lifeline that was improperly secured by tying a knot around a roof truss, rather than using an appropriate anchorage point or device.  L&R Construction of GA LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.502(d)(6)(i), which was contained in OSHA inspection number 1823477, citation number 1, item number 2, and was affirmed as a final order on  12-01-2025, with respect to a workplace located at Lot #3 at 6191 New Bethany Road, Buford, GA 30515.

29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:   a) Lots 18-22, Roof: On or about February 17, 2026, and at times prior, the employer failed to ensure proper use of personal fall arrest systems. Employees were exposed to fall-related hazards, in that the attachment point of their body harnesses was not positioned in the center of the back near shoulder level, as required.  L&R Construction of GA LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.502(d)(17), which was contained in OSHA inspection number 1823477, citation number 1, item number 3, and was affirmed as a final order on  12-01-2025, with respect to a workplace located at Lot #3 at 6191 New Bethany Road, Buford, GA 30515.
