29 CFR  1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used:   a) On February 19, 2026, at the above addressed jobsite, employees were exposed to fall hazards while accessing and egressing the work platform of a frame scaffold without an independent ladder when the working area was approximately 30 inches above the upper most ladder rung. 29 CFR  1926.602(c)(1)(ii): Modifications or additions which affect the capacity or safe operation of the equipment were made without the manufacturer's written approval:  a) On February 19, 2026, at the above addressed jobsite, the employer did not ensure that fork extensions that were utilized on the JLG Telehandler all-terrain forklift were approved by the manufacturer.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
