29 CFR 1910.25(c)(4): Have a minimum width of 22 inches (56 cm) between vertical barriers (see Figure D-8 of this section). On or about February 17, 2026, fixed stairway used for employee access measured less than 16 inches in width, below the required minimum of 22 inches. Employees were exposed to fall hazards and delayed egress during emergency conditions, including fire, due to restricted width and limited maneuvering space.
