29 CFR  1910.184(d): Inspections. Each day before being used, the sling and all fastenings and attachments were not inspected for damage or defects by a competent person designated by the employer. Additional inspections were not performed during sling use, where service conditions warrant. Damaged or defective slings were not immediately removed from service.   (a) On or about February 19, 2026, a damaged six (6) foot long synthetic web sling, that was cut/frayed in the middle of the sling was being used to handle  material  with an overhead crane located in the second crane bay near the shot blast booth.  The sling was not inspected prior to being placed into service, exposing employee(s) to  a struck by and crushing hazard.  (b) On or about February 19, 2026, a chain sling used to attach and lift the 11,000 pound capacity Magswitch magnet lifter with a damaged hook safety latch, was being used on a P&H 40 ton hoist crane in the third crane bay. The  chain sling and hook was not inspected prior to being placed into service, exposing employees to a struck by and crushing hazard.

1910.1200(h)(1)  Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and safety data sheets. (a) On or about February 19, 2026, in house fabricated plate hook attachments used to lift steel plate onto the burn tables by the machine operator had not been rated and marked before use with the attached chain sling. This exposed the employee(s) to a struck-by and crushing hazard.  (b) On or about February 19, 2026, makeshift lifting rings attached as a part of a chain sling and used to lift a 14 yard dumpster, had not been rated and marked for use with the attached chain sling. This exposed the employee  to a struck-by and crushing hazard.
