29 CFR  1910.28(b)(1)(i): Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):   (a) On or about February 20, 2026, and dates prior,  at the Blanchard 117HACD-200-224  rotary surface grinder, the employer did not ensure that employees working from or on the  200-inch diameter horizontal magnetic chuck were protected from falling into the surface grinding machine coolant return area. This exposed employees to a fall hazard of approximately five (5) feet into the bottom of the machine cavity.  Employees are exposed to this fall hazard during machine setup and part loading/ unloading operations, and during maintenance and servicing work tasks.

Employees were exposed to caught-on, caught-in, struck-by, and crushing hazards from the rotating work piece and associated tooling components.  (b) On or about February 20, 2026, at the Blanchard Model 32D--60  Rotary Surface Grinder, the employer did not ensure employees working at or near the point of  operation  and other energized machine parts while working on and around the surface grinding  machine were protected from exposure to the rotating work holding device and attached parts and tooling . Employees were exposed to caught-on, caught-in, struck-by, and crushing hazards from the rotating work piece and associated  adjustable tooling components.  (c) On or about February 20, 2026, at the Blanchard Model: 22HK-42 Rotary Surface Grinder, the employer did not ensure employees working at or near the point of  operation  and energized machine parts while working on and around the surface grinding  machine were protected from exposure to the rotating work holding device and attached parts and tooling . Employees were exposed to caught-on, caught-in, struck-by, and crushing hazards from the rotating work piece and associated  adjustable tooling components.

29 CFR  1910.305(a)(2)(x): Flexible cords and cables shall be protected from accidental damage, as might be caused, for example, by sharp corners, projections, and doorways or other pinch points.  (a) On or about February 20, 2026, and dates prior in the surface grinding area, employees were exposed to trip and fall, and electrical shock  hazards when the energized temporary extension cable supplying 120 volt AC  power,  feeding portable lighting and  equipment was laid on the steel  grated floor work area, were not protected against damage.
