29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: On or about February 22, 2026, employees were exposed to machine hazards associated with moving parts and in-going nip points during servicing and inspection at the B&O Saw, Model 824 to remove scrap metal that had dropped down thought the saw blade opening on the top of the table. The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the servicing work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards. Penn Aluminum International LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR1910.147(d), which was contained in OSHA inspection number 1587206, citation number 01, item number 02 and was affirmed as a final order on 09-27-22, with respect to a workplace located at 1117 North Second Street, in Murphysboro, Illinois.
