29 CFR  1926.102(a)(2):The employer did not ensure that each affected employee used eye protection that provided side protection when there is a hazard from flying objects.  a. On or about March 10, 2026, at LOT 378: employees were exposed to stuck-by hazards while using pneumatic nail gun at 140psi, in that eye or face protection was not used while working on the roof.  Triade Multiservices Inc, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 1817068, citation number 1, item number 1 and was affirmed as a final order on 06-11-2025, with respect to a workplace located at 1302 East Fuller Cross Rd Winter Garden Florida 34787.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a. On or about March 10, 2026, at Lot 378: employees were exposed to an approximately 19-foot 9-inch fall hazard while installing shingles on a roof with a 4:12 pitch, in that fall protection system, such as lanyards were not used during this activity.  b. On or about March 10, 2026, at Lot 378: employees were exposed to an approximately 10-foot 1-inch fall hazard while installing shingles on a roof with a 4:12 pitch, in that fall protection system, such as lanyards were not used during this activity.  c. On or about March 10, 2026, at Lot 378: employees were exposed to an approximately 10-foot 1-inch fall hazard while framing on the second floor, in that fall protection system, such as guardrail were not used during this activity.   Triade Multiservices Inc, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1817068, citation number 1, item number 3 and was affirmed as a final order on 06-11-2025, with respect to a workplace located at 1302 East Fuller Cross Rd Winter Garden Florida 34787.
