29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  6 Elise St, Westfield, MA:  On and before March 12, 2026, the safety interlock system for the Sheeter 2 caged machinery area was inoperative, exposing operator employees who entered the area to hazardous machine operations without utilizing the written energy control procedures. On March 12, 2026, an employee who was repositioning stacks of paper inside the caged area received injuries when a leg became caught in the machine as the automated sliding tables started to move.

29 CFR  1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy:  6 Elise St, Westfield, MA:  On and before March 12, 2026, employees were exposed to injury while performing servicing and maintenance activities on the Sheeter 2 machine as the energy control procedure did not clearly state the steps to control hazardous energy sources. Sections 2.4 and 2.6 differed as to the number of air lines to disconnect and the procedure was not specific as to whether one, two, or three locks were required to isolate the pneumatic sources.

29 CFR  1910.147(c)(4)(ii)(D): The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures:  6 Elise St, Westfield, MA:  On and before March 12, 2026, the energy control procedure for the Sheeter 2 machine, allowed for potential hazardous energy exposure as it directed to perform servicing and maintenance tasks without including the verification steps that authorized employees must take to verify that the lockout devices had effectively isolated the potential electrical and pneumatic energy sources.

29 CFR  1910.147(c)(6)(i):  The employer did not conduct a periodic inspection of the energy control procedures at least annually to ensure that the procedure and the requirement of this standard were being followed:  6 Elise St, Westfield, MA:  On and before March 12, 2026, the employer had not performed a comprehensive periodic inspection of energy control procedures for all multi-energy source machines, at least annually, including the Sheeter 2  Machine, to verify that each procedure was adequate and being properly used by employees.  *Abatement Notes:  A periodic inspection shall include the following elements:  - Appointment of an authorized employee to inspect each local procedure for employee deviations and procedural inaccuracies during an energy control task - A review by inspector with each authorized employee, of that employee's responsibilities under each energy control procedure they are authorized to use - Certification that the periodic inspections have been performed by identifying the machines or equipment on which the energy control procedures were being utilized, the dates of the inspections, the employees included in the inspections, and the persons performing the inspections *Abatement notes are intended only to provide clarity regarding existing requirements under the law or agency policies. Abatement notes do not create (or diminish) legal obligations under the Occupational Safety and Health Act.
