General-duty citation text
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish a workplace that was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to struck-by and caught-between hazards while lifting and positioning castings using a hoist and hook assembly. On or about March 16, 2026, the employer did not ensure that castings were securely attached during hoisting operations in the blast area. Employees used a jib crane equipped with an air-powered hoist and hook assembly to lift and position castings that could slip during normal handling, exposing employees to struck-by and caught-between hazards while guiding suspended castings into position. The employer used a hook assembly as a means of load attachment, which did not provide secure engagement of the load.
