29 CFR  1926.102(a)(1): The employer did not ensure employees used eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  At the temporary jobsite located at 12063/12067/12071 Arvada Blvd. Wichita, Kansas, employees performing framing activities using pneumatic nail guns were exposed to struck-by hazards in that eye protection was not utilized.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  At the temporary jobsite located at 12063/12067/12071 Arvada Blvd. Wichita, Kansas, employees performing framing activities were exposed to fall hazards in the following instances:   a.  At the temporary jobsite located at 12063 Arvada Blvd. Wichita, Kansas, employees performing framing activities were exposed to fall hazards of approximately 13 feet 1 inches from the windowsill to the ground below in that fall protection was not utilized.  b.  At the temporary jobsite located at 12067 Arvada Blvd. Wichita, Kansas, employees performing framing activities were exposed to fall hazards of approximately 11 feet 6 inches from the second story floor joist to the ground below in that fall protection was not utilized.  c.  At the temporary jobsite located at 12071 Arvada Blvd. Wichita, Kansas, employees performing framing activities were exposed to fall hazards of approximately 9 feet 10 inches from the top plate to the cement floor below in that fall protection was not utilized.

29 CFR  1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:  At the temporary jobsite located at 12063/12067/12071 Arvada Blvd. Wichita, KS 67235, an employee performing framing activities was exposed to fall hazards in that the employee was allowed to straddle the top of the a-frame ladder and step on a support rung that is not designed as a step.
