29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   a) Church roof - 334 W. Greene St., Monticello, GA: On or about March 26, 2026, the employer exposed employees to struck-by hazards in that, the employer failed to ensure that employees were protected by protective helmets while they were working below overhead roofing operations on the ground.

29 CFR  1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    a)Church roof - 334 W. Greene St., Monticello, GA: On or about March 26, 2026, the employer exposed employees to a fall hazard of approximately 20 feet to the lower ground level, in that the employer failed to ensure appropriate fall protection systems were provided to employees removing and installing shingles on the roof.

Greene St., Monticello, GA: On or about March 26, 2026, the employer exposed employees to fall hazards of approximately 20 feet to the lower ground level, in that employer failed to ensure ladders used to access the roof of the structure were extended at least three feet above the upper landing surface.
