29 CFR  1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:(a)(LOCATION)(SPECIFIC OPERATION(S) AND/OR CONDITIONS)   a) Basement: An employee was exposed to head injuries while working below employees that were hammering and using pneumatic nail guns to install floor joist. The exposed employee was not wearing any type of head protection, on or about 4/8/26.

29 CFR  1926.102(a)(1): The employer did not ensure employees used eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a)  1st Floor:  An employees was exposed to eye injuries while using a pneumatic nail gun to install floor joist on the 1st floor, on or about 4/8/26  b) 1st floor: An employees was exposed to eye injuries while sticking floor joist with a hammer  and was not wearing eye protection, on or about 4/8/26

29 CFR  1926.1053(b)(4): Ladders were not used for purposes other than the purpose for which they were designed:  a)  262 Passaic Street, Passaic NJ/Basement:  Employees were permitted to use a 8 foot fiberglass stepladder in the closed position leaning against the first floor joist coming up from the basement, to be use as an access to the 1st floor. on or about 4/8/26.

29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a) 262 Passaic Street Passaic NJ:  Employees were exposed to fall hazards of approximately 7' feet from while installing floor hoist without the use of personal fall arrest system or any other means of fall protection, on or about 4/8/26.   The GM Glenn LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1926.501(b)(1), which was contained in OSHA inspection number 152198, citation number 1 item number 1 and was affirmed as a final order on 4/16/26, with respect to a workplace located at 11 Bennett Street Jersey City NJ 07304.
