Libbey Glass, Inc. located at 940 Ash Street in Toledo, Ohio;  On or about March 27, 2026, the employer did not ensure that the required injury or illness description information was provided on the OSHA Form 300 for calendar year 2026. Recordable injuries occurring on or about January 12, 2026 and February 26, 2026 did not include the description of injury, part of body affected and the object/substance that directly injured the person under column F of the OSHA Form 300.

Libbey Glass, Inc. located at 940 Ash Street in Toledo, Ohio;  On or about March 22, 2026, the employer did not ensure the establishment's OSHA Form 300A data for calendar year 2025 was submitted through the Injury Tracking Application (ITA) by March 2, 2026.
