OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees On or about April 21, 2026 and at times past, at 2642 Meeker Dr. Baton Rouge, LA 70814, employees was exposed to the recognized hazard of falling from an elevated work platform while operating a Crown high?lift powered industrial truck by not wearing the required body harness and without attaching the tether to the manufacturer?designated anchorage point.

29 CFR 1910.178(l)(1)(i): The employer did not certify that each operator had been trained and evaluated as required by this paragraph (l): On or about April 21, 2026 and at times past, at 2642 Meeker Dr. Baton Rouge, LA 70814, inside of the warehouse, employees were operating a powered industrial truck to move and store auto parts on and off shelving. The employees were permitted to operate the truck without having completed the required training and evaluation.
