29 CFR  1926.502(d)(15)(ii): Anchorages used for attachment of personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, and not installed under the supervision of a qualified person.   Site- On or about June 5, 2026 employees using personal fall arrest equipment while working from the roof were not protected from fall hazards where anchor points for personal fall arrest systems were not installed under the supervision of a qualified person. Multiple anchor points were attached to the roof with two life lines attached to the same anchor point for use by multiple employees, anchor points were installed without filling the appropriate amount of holes with fasteners, and the anchor points were turned 90 degrees from the intended orientation.
