29 CFR  1926.102(a)(1): The employer did not ensure employees used eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  On or about June 24, 2026, at 38120 Fox Run Drive, Solon, OH 44139, the employer failed to ensure employees operating a pneumatic nail gun to attach weatherproofing while working from a residential roof wore eye/face protection.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about June 24, 2026, 38120 Fox Run Drive, Solon, OH 44139, the employer failed to ensure employees performing weatherproofing on a residential roof were protected from a fall of approximately 20 feet from a 4:12 pitch in that no personal fall arrest systems/devices were utilized.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about June 24, 2026, at 38120 Fox Run Drive, Solon, OH 44139, the employer failed to ensure employees utilizing a portable ladder to gain access to a residential multi-story house roof were protected from a fall of approximately 20 feet to the ground by ensuring all ladders were extended three [3] feet above the landing.
