Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The

free from recognized hazards that were causing or likely to cause death

chlorination unit. Feasible engineering and work practice controls
which minimize potential employee exposure, such as but not limited
to line sampling devices, covered/closed tranfer containers, and/or
double valve disposal units had not been implemented.
