General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to an amputation hazard: a) The foot-operated tripping controls (foot pedal or treadle) on the Corner Cutting Machines (#5974, #3, Schuch Machine, Knowlton #5016, Double Corner Cutter (Duplex #216) and the large Hobbs Double Cutter (treadle) were not guarded to prevent accidental actuation and the point of operation of these machines were not guarded, March 19, 1987. Among other methods, some feasible and acceptale abatement methods to correct these hazards are: 1) Install a barrier guard along the length of the treadle or enclose the foot pedal to prevent unintentional activation. 2) Install pedal or treadle locking device and enforce the use of such device. 3) Install point of operation guard on all machines to prevent unintentional entry into the machine operating zone.
