On 9/9/2016, at approximately 1:30 PM, an employee and coworker were trying to uncrate electrical switchgear. They were using tools to pry the switchgear's cabinet off the wooden pallet when the switchgear shifted over onto the employee's right foot and knocked him backwards, leaving him hanging over the mezzanine by his caught foot. When his coworker righted the cabinet, it freed his foot, causing him to fall head-first to the concrete floor below. He suffered skull fractures, right leg/ankle fractures, and shoulder and arm injuries, requiring hospitalization.
