On 12/9/2016, at approximately 9:10 a.m., an employee was walking to a conference room for a meeting. As she exited the elevator, she slipped and fell on the wet tile floor, which was being mopped at the time. She suffered a fractured tailbone and a hematoma, requiring hospitalization.
