On November 7, 2017, an employee was driving a lift truck up a loading ramp while another driver was about to come down the ramp. The other driver stopped and signaled the employee to come up the ramp. The injured employee was trying to pull the truck to the side of the ramp when the wheel came off the ramp. He tried to raise the clamp-up boom to move the lift truck forward, but it started to tip over. He then jumped off and fell to the ground. The fork truck came off the ramp and the cage struck him in the back. He suffered a fractured hip, back and lower extremity injuries, and internal bleeding.

An employee was operating a forklift to move panels around the dock area when the vehicle struck a joint where the two floors meet. The employee lurched forward and struck his head on the steering wheel. The employee then fell from the vehicle and sustained a dislocated disc in their back, requiring hospitalization.
