An employee was on a ladder testing an energized wire in the ceiling to make it safe when his pliers touched the electrical junction box, causing an arc flash. The flash startled the employee, causing him to fall off the second rung of the ladder to the floor, re-aggravating a pre-existing shoulder injury.
