An employee was working on the ground at an active job site when a reversing milling machine ran over the employee's left foot. The machine continued running up his leg to approximately his hip. His left foot and leg were crushed, with damage to the left hip and amputation of the left leg from the knee down. He also suffered a fractured right leg and hip.
