Two employees were moving two pin protectors to another location to weigh them. They were trying to stand the two pin protectors up on end to secure them to a forklift with a chain and bolt. As they stood the pin protectors up, they attempted to pull slack out of the chain. One employee accidentally bumped heads with the injured employee, which caused him to stumble and lose his grip as he put his hand on the ground to balance himself. At that point, the pin protectors fell over onto his finger. He immediately pulled his hand out from under the pin protector, resulting in a right ring finger amputation.

The injured employee tried to catch a co-worker who was having a seizure. Both employees fell to the floor and the injured employee sustained a broken hip.

Kings Island Park LLC

An employee was cleaning the park and sweeping outside of the front gate. A guest bumped into the employee, causing her to fall and sustain a hip and wrist fracture.

Presidente Supermarket

An employee was near a co-worker who was standing on stacked milk crates stocking products. The co-worker began to fall and as the injured employee went to help, her co-worker fell on top of her. The injured employee was hospitalized with a hip fracture.
