An employee was moving rail cars. While standing on the rail car trying to set the brakes, he slipped and fell and the rail car rolled over his foot. The employee's foot was surgically amputated.

An employee was riding on the end platform of a tank railcar. As the railcar came to a stop, the employee fell off the platform to the ground. The railcar wheels ran over his forearm as he fell. The forearm was medically amputated.

An employee was in the process of switching railcars in the scrap yard when he slipped and fell from the engine landing beneath the railcar wheel as the railcar was moving. His right leg and left foot were crushed, resulting in a right leg amputation.
