At about 6:00 p.m. on March 7, 2019, an employee was performing preventive maintenance on fans/belts on the HVAC system. He was working on a vertical stack of components that included four belts, working from top to bottom. The bottom belt pulled in his left hand, causing the partial amputation of three fingers. He was hospitalized.
