Final narrative

A forklift was lifting a block cart so an employee could hit the caster wheel with a "dead sledge" in an attempt to straighten the wheel. Due to the striking of the caster wheel, the block cart shifted and slipped on the forks, which caused the base plate to slide off the block cart. The employee lost his balance and fell to the ground. The base plate slid and struck the employee's left leg causing a broken left femur.
