Final narrative
On 8/19/2020, at 6:20 a.m., an employee was performing routine cleaning on a reclaim pelletizer. While performing this task, the employee attempted to activate the dump valve that stores hot water. Due to an internal defect on the pipe valve, the hot water did not drain out completely. As the employee released the locking collar, the stored water released through the gap, spraying hot water onto the employee. The employee sustained first and second degree thermal burns to the chest and left arm.
