An employee was working on fitting and welding a beam 3.5 feet off the ground. The employee went to walk across a skid to get to the other side of the beam and their foot got caught, causing them to trip and fall to the ground. They lost two front teeth and sustained fractures to both hands. The employee was hospitalized.
