An employee stepped onto a piece of oriented strand board (OSB) that was covering a hole in the bottom of a furnace casing. He fell through the board, landing on the floor at the bottom of the structure, about 12 feet below. He suffered a broken left ankle, a broken right little toe, and a sprained left wrist.
