The injured employee was being trained as a spot welder. He went to move an adjustment knob on a hydraulic cylinder to bring the tips of the welder closer together. The welding machine actuated and the injured employee's left thumb was crushed by the hydraulic cylinder resulting in an amputation from the top knuckle to the thumb tip.
