Final narrative
An employee had been operating an excavator from the safety platform and was putting tree limbs onto the belt of a horizontal grinding machine. A limb got stuck in the grinder. Employees raised the anvil and the limb was removed. When the anvil was being lowered back down, the employee's right foot got crushed between the top of the anvil guard and the grinder frame. They sustained two toe fractures and a partial amputation to one toe.
