Three maintenance employees were replacing an impression roll (anilox roll) on the first print section of the rotary die cutter. The employees were using an ATV jack to support the operator side of the roll while trying to position it into the machine. As the roll and jack moved, the roll fell from the jack (approximately 4-6"). The injured employee tried to catch the roll and their left ring and middle fingers were caught between the shaft on the roll and two 4" x 4" wood planks. The employee sustained injuries to the fingers, resulting in surgical transphalangeal amputations to the fingers.
