An employee was loading a delivery truck. The employee had a pallet on the forks of a forklift about 3" off the ground. He was moving the pallet with 400 pounds of freight upright after it tipped, when the pallet came off the forks and his left hand was caught between the freight and the floor of the trailer. His middle and ring fingers were crushed and medically amputated.
