Employees were changing out the forks to a picker attachment/device on a hydraulic forklift. Debris was stuck in the side mast holes that are for the locking pins. The injured employee cleared debris from the left side and then moved to the right side. The locking pin extended while the employee was clearing the debris, resulting in a left index finger amputation.
