An employee started the clean-in-place (CIP) cleaning cycle on a filler machine. They noticed there was no pre-rinse water flowing to the filler return tub. Upon troubleshooting the issue, the employee ascended a mobile platform (48 inches high) so they could access an end cap which was tri-clamped to a 2 stainless steel line. As they loosened the clamp, pressure in the line caused the end cap to blow off. The 2 stainless pipe struck the employee's face and caused them to fall off the platform. The employee was hospitalized with a fractured nose and top jaw, two black eyes, and a swollen face.
