An employee was standing on a ladder, using a chipping hammer to remove a concrete masonry unit block wall from around ductwork. Below the block he was removing, a steel lintel above a door became loose. As he was steadying the lintel, a concrete block fell onto his left middle finger. The fingertip was amputated.
