Final narrative
Employees were installing a projector on the ceiling of an auditorium using a scaffold. While the injured employee was getting down from the scaffolding, it became top-heavy and tipped over. The injured employee fell to the floor, landing on his left leg and suffering a broken left fibula and tibia.

An employee was walking and taking post-locate photos. He stepped on a ceramic drainage tile and his foot/leg punched through the tile. His leg was lacerated by the broken tile. The employee was hospitalized with internal bleeding due to a punctured femoral artery.

Two employees were constructing a 30-foot-tall aboveground storage tank, fitting a double roof on the tank. Half of the roof had been stitch-welded to the tank, and the employees were fitting/tacking down the second half of the roof, when the roof shifted and fell in. One employee fell and sustained fractures to their right foot and shin. The second employee suffered a broken back. Both employees required hospitalization.

An employee was on a ladder checking the fuel level in an oil tank. As they started to descend the ladder, the ladder slid across the fuel tank and the employee slipped and fell off the ladder. Their right little finger was caught between the ladder and a metal stanchion when the ladder struck the floor. The employee sustained an avulsion to their right little finger and a laceration and abrasion to their right arm.

An employee was on a mobile scaffold to remove a drywall partition when the partition fell onto the scaffold, causing it to fall. The employee also fell to the ground and sustained a torn hip muscle and a hernia.
