An employee was putting up steel panels on the roof of a building. The employees was using a standing seam roof anchor and a rope with a rope grab. The standing seam anchor came loose from the metal which allowed the employee to free-fall off the roof. As the employee was sliding down the roof, his feet got caught in the gutter which turned him upside down. He sustained a concussion, a severe laceration to his head, and eight fractured ribs.
