On [REDACTED] at approximately [REDACTED] an Extrusion Operator was being trained on how to perform a changeover on Rack 24 (2+2 Rack) using the new safety hook knife.. As the operator attempted to cut the tubing the knife got caught in the tubing. This resulted in the knife pulling against the operator’s right index fingertip. The operator sustained a bruise and a minor laceration to her right index fingertip (no stitches). The nature of the injury and the design of the knife indicate that the knife “hook” not the blade caused the injury.
