Before the incident
The card press operator in advance of making an adjustment to the [REDACTED] Unit pulled the clear plastic curtain guard up and out of his way by hanging it on a nearby hook.
What happened
While the operator was making the adjustment to the Telstar Unit he inadvertently bumped into the clear plastic curtain and jarred it loose from the hook. This caused the curtain to fall onto the operator 's head. The curtain has an aluminum bar that is fastened to the bottom edge of the curtain with small screws one of the screw threads hit the operator in the head when the curtain fell causing a cut to the left side of the operator 's scalp.
